Dive Consent Forms

If you're wanting to join us at Kai Iwi Lakes for our February 2022 Freedive camp you'll need to make sure you've completed these forms before you commit.

a medical clearance questionnaire, and consent form). These need to be filled out BEFORE anyone commits so i want to get this available on the website before we go live with spots for the course. I've attached them below. Can we have them on the webby somewhere easy to find? Maybe even their own little tab "Dive Consent forms"  or something.
